The 5.9L Cummins earned its reputation the hard way: hundreds of thousands of miles pulling trailers, feeding cattle and hauling campers. The youngest of these trucks is now close to 20 years old, and the ones still running strong have owners who stayed ahead of a short list of known problems. This guide covers that list for every 5.9, what to buy, and what order to do it in.
The 5.9 used three completely different fuel systems in 14 years, so a part or service interval that is right for one truck can be wrong for another. Pick your year range to show only the maintenance and parts for your truck. Every product card lists the fitment exactly as it appears on the product page, and the product page has the final word.
Alligator splits the 5.9 into the same four year ranges the parts catalogs do, because that is where the parts change. The 1998 model year is split between two different engines, so check yours before you order.

Open the hood and look at the driver side of the engine. A 12V has a long inline injection pump with six steel lines coming out of the top in a row. A 24V has a compact rotary VP44 pump with an electronic control unit mounted on top of it. The 8th character of the VIN is often cited as "D" for the 12V and "6" for the 24V, but confirm it with a VIN decoder before you order parts. Many 24V trucks also wear a "24 Valve" badge, but badges get swapped, so trust the engine.
The injectors changed in the middle of 2004, and the two designs are not interchangeable. Running the wrong set causes rough running, high exhaust temperatures and possible engine damage. Check the engine data tag on the valve cover before ordering injectors or anything else listed for only one of these ranges.
Factory intervals were written for new trucks. Most 5.9s are now 20 to more than 30 years old with unknown history, so we recommend the severe column for any truck that tows, idles a lot, runs dirt roads, or has more than 200,000 miles. Your owner's manual, an oil analysis and your mechanic have the final word.

Behind the timing cover, a small dowel pin locates the gear housing to the block. On 12-valve engines it can work loose and drop into the timing gears. The best case is a cracked timing cover. The worst case is a damaged gear train and a truck on a trailer. The fix is a small tab that bolts next to the pin and traps it so it cannot back out. You have to pull the timing cover, so replace the timing cover gasket and look at the front crank seal at the same time. If you bought the truck with no records, assume it has not been done.

BD tab, bolt and tooling to lock the dowel pin in place on 1994-1998 12-valve engines. The most important part on this page for a 12V.

Check price and fitmentOn 1994-2002 trucks a gear-driven vacuum pump behind the power steering pump supplies the brake booster. Its shaft seal commonly leaks oil. A reseal kit is inexpensive. Index the drive gear carefully when it goes back together.
The VP44 is cooled and lubricated by the fuel flowing through it, and about 70 percent of the fuel it receives goes back to the tank to carry heat away. When the lift pump gets weak, the VP44 runs hot and starved. The pump-mounted computer's solder joints fatigue with heat, and the pump fails. The classic symptoms are long cranking on hot restarts, stalling, and sudden loss of power. Many original VP44s were killed by a lift pump that nobody was watching.
Supply pressure should never drop below 5 psi, at idle or at full throttle under load, and 12-15 psi is the target. The only way to know is a gauge that stays on the truck. The factory electric lift pump is mounted on the block, where heat and vibration shorten its life. A frame-mounted lift pump with extra filtration, or an in-tank pump, is the standard fix.

Check price and fitmentSome 1999-2001 24-valve blocks with the number "53" cast into the side developed cracks in the water jacket on the passenger side, just below the freeze plugs. Look for "53" cast into the side of the block, usually on the passenger side and sometimes on the driver side, so check both. Then inspect the passenger side for coolant seepage or a crack. On automatic trucks the transmission cooler can hide the area, so look carefully. Heavy towing, overheating and skipping warm-up raise the risk. A crack is repaired with a cast iron stitching process such as Lock-N-Stitch, often with the engine still in the truck. Welding cast iron makes it worse.
The dowel pin is primarily a 12-valve problem. On 24-valve trucks it is low risk, but the tab is cheap insurance any time the timing cover is off. The Industrial Injection dowel pin kit is listed for 1994-2002 engines.
An intermittent dead pedal or surging on 1998.5-2002 trucks often traces to the APPS. Codes P0121, P0122, P0222 or P0223 point to it.
Dirt and water are what kill common rail injectors. Early 2003-2007 injectors often needed attention between 100,000 and 150,000 miles. Warning signs include hard or no starts, a rough idle, a knock, white vapor from the tailpipe at startup, fuel in the oil, and heavy blow-by from the oil fill. A return flow test settles it: excess return flow means worn injectors.
Replace injectors as a set and always install new connector tubes with them. Remember that 2003-2004 and 2004.5-2007 injectors are different designs and are not interchangeable.

Below about 5 psi the CP3 is not getting enough fuel. On 2003-2004 trucks the factory pump is mounted on the block. Many were moved into the tank under warranty, and 2005-2007 trucks came with the pump in the tank. Lift pump kits are sold in versions for both, so check yours before ordering. Check price and fitmentA rough idle, low power or a no-start can come from the fuel control actuator (FCA) on the CP3. If the truck starts only with the FCA unplugged, it is the likely culprit. A rail pressure relief valve that sticks open keeps the rail from building the pressure it needs to start.
The 12V's P7100 pump is mechanical and famously tough, and it tolerates weak supply pressure far better than the VP44 on later trucks. That tolerance is why many owners never check it. But the factory lift pump is a cam-driven mechanical pump that is decades old on most trucks, and a weak one shows up as lost power on long pulls and hard starts. Tee a gauge into the line feeding the injection pump and check pressure at idle and under load. Change the fuel filter before you blame the pump, since a clogged filter looks the same.

Change it every 15,000 miles, or 7,500 when towing, and drain water from the separator at every oil change.
The factory banjo bolts in the fuel supply line are small and restrictive. Upgraded banjo bolts are a cheap way to help the lift pump keep pressure up. Injectors on the 24V wear like any other. For a work truck, replace them as a matched set at or near stock flow. The set we list below is a performance step, so call our sales team if you want a stock flow set.

Check price and fitmentEvery 5.9 holds about 12 quarts with the filter. A quality heavy duty 15W-40 diesel oil is the right choice for most trucks, and 2003-2007 trucks can also run a 5W-40 full synthetic in any temperature. Older engines with more blow-by benefit from the shorter end of the interval range. An oil analysis once a year catches coolant or fuel in the oil early, which matters most on a 53 block 24V or a common rail truck with tired injectors.
The air filter matters as much as the oil filter on a dusty ranch road. Replace it based on the restriction indicator or inspection, not mileage, and consider a pre-filter wrap on an aftermarket intake.

Check price and fitmentMost 5.9 cooling problems come down to age: hardened hoses, weak radiator caps, tired water pumps and thermostats that no longer open on time. If you tow in heat, cooling is where your reliability is won or lost. Use a 50/50 mix of a quality antifreeze and distilled water, never mix coolant types, and test it with strips. Coolant additives wear out, so test the protection level, not just the freeze point. Check the water pump weep hole every 15,000 miles.
On 12-valve trucks, the coolant bypass and heater lines are common age-related leak points, and the stock thermostat is 180°F. Stainless braided replacement lines end the leaks for good. A lower temperature thermostat helps trucks that tow in hot climates. In cold climates, stay with a stock temperature thermostat.

Check them at every oil change and pressure test the system if power falls off. The long one-piece exhaust manifold on the 5.9 also cracks from heat cycling. Check price and fitmentA stock 5.9 rarely needs a head gasket. Trucks that tow heavy with added fuel are a different story. If the head comes off for any reason, install head studs. They clamp the head more evenly than factory bolts and turn a recurring repair into a one-time job. Valve lash is the other engine job owners skip: 0.010 in. intake and 0.020 in. exhaust, set with the engine cold.

Check price and fitmentThe front end is the most common reason owners fall out of love with an otherwise great Ram. Worn track bar bushings, drag link and tie rod ends, ball joints, unit bearings and a steering box that flexes the frame all add up to wandering, loose steering and death wobble. On many high mileage Rams, the front end is worn out. Fix the parts, not the symptom.

Synergy heavy-duty steering kit. Replaces the drag link and tie rod in one package. Listed for 1994-1999 4WD.

Check price and fitmentThe 4-speed automatic is the weak link behind any 12V that tows. Change the fluid (ATF+4) and filter and adjust both bands on schedule. On 1996-1998 47RE units, erratic shifting or no upshift is often the governor pressure solenoid or sensor, not a failed transmission. If it is already slipping, service will not save it, and a rebuild with upgraded parts is the right call.

ATS governor pressure solenoid conversion. Fixes a common 47RE shifting problem.

Check price and fitmentThe NV4500 is close to bulletproof, but clutches wear and the hydraulics fail with age. Use a 75W-90 that meets the Syntorq spec. The wrong fluid ruins synchros. Lugging in 5th gear with a trailer can loosen the 5th gear nut, so downshift on hills.

Check price and fitmentChange the ATF+4 and filter and adjust both bands every 30,000 miles, or every 15,000 if you tow. Starting in 2nd gear, harsh or delayed shifts, or no overdrive often trace to the governor pressure solenoid or sensor inside the pan. Replace both together with a fluid and filter service. A deep pan adds fluid capacity and helps with heat.

Check price and fitmentIf the transmission is already slipping under load, a rebuild with upgraded parts is the right answer. SunCoast's 47RE kits come in categories by power and use, starting with a Category 1 kit with converter for stock towing trucks.
The NV4500 takes a 75W-90 that meets the Syntorq spec. The NV5600 six-speed, offered from 1999 and the only manual behind the 2001-2002 High Output engine, takes Mopar manual transmission fluid. The two are not interchangeable. If the clutch pedal feels soft or the clutch will not fully release, check the hydraulics first.

Check price and fitmentThe 48RE is stronger than the 47RE, but heat is still its enemy. The factory interval is long, so if you tow, change the ATF+4 and filter and adjust the bands every 30,000 miles. A deep pan adds capacity and cooling. Governor pressure problems show up as erratic shifting or starting in 2nd. A stock 48RE can slip once power is added.

Check price and fitmentThe NV5600 was used through 2005, then the G56 took over during the 2005 model year. They take different fluids: Mopar manual transmission fluid in the NV5600, ATF+4 in the G56. The factory dual mass flywheel does not handle added power well, and many owners replace it with a single mass clutch kit.

Call 208-719-7400Send us a messageLook at the injection pump on the driver side of the engine. A long inline pump with six lines in a row is a 12V. A compact rotary VP44 pump with an electronic module on top is a 24V. The 8th character of the VIN is often cited as D for the 12V and 6 for the 24V, but confirm it with a VIN decoder.
About 12 quarts with the filter on every 5.9 from 1994 to 2007. A heavy duty 15W-40 diesel oil is the common choice, and 2003-2007 trucks can also run a 5W-40 full synthetic.
Supply pressure should never drop below 5 psi at any time, including full throttle under load, and 12-15 psi is the target. Keep a gauge or a low pressure alarm on the truck so you know.
Long cranking on hot restarts, stalling, sudden loss of power and a P0216 code. Check lift pump pressure first, because a weak lift pump is the most common reason VP44s fail.
Look for the number 53 cast into the side of the block, usually on the passenger side and sometimes on the driver side. Only 1999-2001 24-valve blocks with that casting have a documented cracking pattern, on the passenger side below the freeze plugs. If you find one, check it regularly for coolant seepage.
The dowel pin is primarily a 12-valve problem. On a 24V it is low risk, but the tab is inexpensive insurance any time the timing cover is off.
1998.5-1999 and 2000-2002 trucks use different cartridges, and 2003-2007 trucks use a 7 micron filter. Do not use the 2003-2007 filter on a 2000-2002 VP44 truck, even though it can fit.
No. They are different designs and are not interchangeable. Running the wrong set causes rough running and high exhaust temperatures. Check the engine data tag on the valve cover before ordering.
On 2003-2004 trucks the factory pump is on the block, though many were moved into the tank. On 2005-2007 trucks it is in the tank. Lift pump kits are sold in versions for each, so check yours first.
The NV4500 takes a 75W-90 that meets the Syntorq spec, the NV5600 takes Mopar manual transmission fluid, and the G56 takes ATF+4. They are not interchangeable, and the wrong one can ruin synchros.
0.010 inch intake and 0.020 inch exhaust, set with the engine cold, on both the 12-valve and 24-valve engines.
Start with the track bar and its bushings, then the ball joints, tie rod ends and drag link, then align the truck. A steering stabilizer hides the symptom but does not fix the worn parts.
